How does my insurance work if I am a member?

Your membership fee pays for your Annual Wellness Program as well as all of your office visits with the physician. You will still need your insurance to pay for any labs that we send out or any outside diagnostic tests we order such as MRIs. If you wish to see the dietitian or physical therapist for additional visits beyond the once yearly assessment you can use your insurance to pay for those visits.

My insurance already pays for an annual physical exam. Why do I need the Advanced Wellness Program?

Your Advanced Wellness Program is just that--advanced. The program includes much more than a typical physical exam that your health insurance normally covers. We will focus on preventative care, using cutting edge technology to detect risk factors and metabolic disorders. You'll have one-on-one consultations with not only your physician, but other highly trained health professionals including a dietician, a physical therapist, a personal trainer, and a massage therapist. You will leave with a packet of information including your test results and an individualized plan based on your results and consultations. The program's purpose is to ensure that you are well--and that you stay well.

Does my insurance cover my membership fee?

No. The membership includes services not typically covered by your insurance and is not reimbursable by insurance.

Can I use my HSA or FSA to pay for the membership fee?

Yes. You can pay your Advanced Wellness Program membership free with your health savings account, flexible savings account and medical savings accounts. You are welcome to use your HSA checkbook, credit card or debit card to pay. If you have a health reimbursement account, your physician can provide you with a statement of services upon request, so you can seek reimbursement through the claim submission process.

Is my membership fee tax deductable?

For patients who itemize medical expenses on their tax returns, the membership fee may be deductible. For specific questions, check with your tax advisor.

Can I get a refund if I want to cancel my membership?

If you have not yet recieved the Advanced Wellness Program, you may be eligible to cancel your membership. If you have received the services provided by the Program, you cannot cancel your membership until the 1-year contract has expired.

What happens if the 500 membership spots are already filled?

Memberships are filled on a first-come, first-served basis. When our practice reaches our membership limit and additional patients would like to join, they will be placed on a waiting list.

What happens if my physician is away on vacation or unavailable?

Your physician will arrange for coverage and you will recieve contact information for the substitute. In addition, your physician will respond to patient phone calls and emails while they are away.
